looks like it comes with the growing pains of MD as they expand (a little too quickly) holes and gaps tend to happen and get bigger and mistakes aren't caught until it's too late it's a trend with many growing companies when the focus gets put into growth and profit and not enough in QA/QC (most companies almost expect that the issues and holes will sort themselves out, which I believe is a very subtle and toxic perspective to have in any establishment)
