Had to get a replacement because as I suspected the quick disconnect didn't last. The flashlight fell off my keychain twice and I can't risk losing a flashlight this expensive. The replacement has issues with quickly turning on and changing modes as well. It also has a vastly different feeling quick disconnect. It's much harder to twist to the left and disconnect. I have the v2 copper version.
Massdrop handled the replacement so well I am more inclined to deal them in the future.
Edit: the problem seems to be the place where the spring is welded together. It bends outwards and eventually the disconnect will fall off when if it pivots on the flashlight even the slightest.

SpinuchI noticed when mine fell off a couple of times that I didn't get it securely back on after using it. This may not apply to what you experienced, but anyway I check it more carefully now when I put it back on, to make sure it's seated and locked securely.
