How about IEMs with an over the ear hook, like Anker SoundBuds Curve? Granted, you may not like the sound of that particular model, but you get the idea. Have you tried any triple flange tips? Custom IEMs are of course the best way to go.

joeftanI didn't try the hooks per se, but I tried doing the same idea using the cable, if you know what I mean. As far as I figure, those hooks may only keep me from losing the earphone entirely, but it definitely can't keep them in for solid isolation. Gonna try the triple flange, thanks for suggestion.

dmedvinskyI hope the triples work for you. If you do pursue custom fit IEMs, some of the least expensive ways are probably either Etymotic IEMs with Sensaphonics’ custom sleeves, or MEE Audio custom fit sleeves, with a MEE IEM of choice. The MEE Audio option is even less expensive if you can get to a MEE Audio booth and get your ears scanned. Just a couple of other options.
