Which gain is appropriate for my 100 impedance headphones, II or III?

Put it at the lowest gain you can to have the right volume. The higher the gain switch the more chance there is hum.

Depends on the sensitivity of your headphones and the power output at that impedance.
ArtismoAlso out SE out vs balanced out as the gain is 0 gain for SE but 2x gain on xlr in medium gain (II). The other gains also differ, owning this with sources between 2 and 4 volts I recommend gain 2 for a good percentage of popular headphones. I saw someone with IEM's running 4 volt balanced source via the XLR output in HIGH gain on the THX 789, that is not a good idea people. 6.6x (16db) gain and 6W is how you kill IEM's.
